It really depends on the size of the facility. The bigger the facility the bigger the risks taken, guided by the QM team and policies & guidelines, in smaller MD offices, with minimal guidelines and P&Ps...no risk mgt. therefore...lower QM standards. Usually offices can't afford or don't utilize Risk Mgt. nor QM because it's too expensive! Hope this helps.
